Resistant hypertension: a practical clinical approach

Resistant hypertension (RH) is defined as an uncontrolled office blood pressure (BP) despite the use of at least three antihypertensive drugs. With an increasing prevalence, RH implies in a very high cardiovascular risk and needs a careful clinical approach, aiming to control BP and to reduce its morbidity and mortality. The initial diagnostic approach involves drug adherence checking and the evaluation of antihypertensive scheme, emphasizing the use of diuretics and adequate combination and dosages of the two other drugs, which preferentially reduces cardiovascular risk and promotes prevention/regression of target organ damages. Extracellular fluid (ECF) is associated with blood pressure, but reports on the status of the ECF volume in hypertension have been inconsistent. The aim of this study was to assess the ECF volume status in hypertensives with regard to body size in a large cohort. We performed a single-center case–control observation study for patients who visited the outpatient hypertension clinic and health examination center. Ambulatory blood pressure in atrial fibrillation: an irregular conundrum of rate and rhythm

The variability of heart rate and stroke volume in patients with atrial fibrillation (AF) has engendered a scientific dilemma for blood pressure (BP) measurement. However from a clinical standpoint, ambulatory blood pressure measurement (ABPM) provides a perfect solution; an average of BP (and hence the variability) across the 24-h period. ABPM techniques avoid observer bias, can identify non-dippers and white-coat hypertension and also have proven prognostic implications in patients with sinus rhythm. [Actualizado: 9 de octubre 2013].

The aim of this study was to test whether ambulatory blood pressure monitoring (ABPM) in elderly patients with atrial fibrillation (AF) is as feasible and reliable as ABPM is in patients with normal sinus rhythm (SR). Studies of ABPM in the elderly remain limited, and the use of this method in patients with AF remains controversial. The Italian SIIA 2008 guidelines consider ABPM ‘absolutely contraindicated’ for AF patients. This study was conducted on 200 hospitalized patients aged greater than or equal to65 years (68% females; mean age 82.4±6.3 years): 100 patients with SR and 100 patients with permanent AF. Aunque el envejecimiento de la población es en gran parte la causa del aumento de la prevalencia global de hipertensión arterial, los cambios en el estilo de vida (sedentarismo, obesidad, aumento de sal en la dieta, etc.) han hecho que también haya cada vez más hipertensos a edades más tempranas. Así, parece necesario analizar específicamente cómo es el manejo y el grado de control de la presión arterial (PA) de este grupo de sujetos en España. El estudio PRESCAP 2010 (PRESión arterial en la población española en los Centros de Atención Primaria) se diseñó para conocer el grado de control de la PA en una amplia población hipertensa asistida en atención primaria y tratada con fármacos.
